L-carnitine is an amino acid produced by the body that’s also found in food and supplements. Some studies suggest it may offer some health benefits, including increased weight loss, improved brain function, and more. In cells, it helps transport fatty acids into the mitochondria, where they can be burned for energy. This may also help

Methylated Vitamin B12, commonly known as methylcobalamin, is a crucial vitamin needed for a number of metabolic processes in the body.  Methylated means “already broken down” into its Most absorbable form. This water-soluble vitamin is essential for the production of red blood cells, RNA, and DNA. Also, it is crucial for the health of the brain and the proper operation of the neurological system. Methylcobalamin has been more well-known recently as a nootropic vitamin because of its propensity to improve mental clarity, focus and cognition.

Vitamin B12, also known as cobalamin, is an essential vitamin that your body needs but cannot produce.

It’s found naturally in animal products, but also added to certain foods and available as an oral supplement or injection. Vitamin B12 has many roles in your body. It supports the function of your nerve cells and is needed for red blood cell formation and DNA synthesis.

Vitamin B6, also known as pyridoxine, is a water-soluble vitamin that your body needs for several functions. It has health benefits for the body, including promoting brain health and improving mood. It’s significant to protein, fat and carbohydrate metabolism and the creation of red blood cells and neurotransmitters.

Your body cannot produce vitamin B6, so you must obtain it from foods or supplements.

Niacinamide is one form of vitamin B3 (niacin) that plays an important role in energy metabolism and cell health. It may offer benefits related to skin care and skin cancer, as well as chronic kidney disease and type 1 diabetes.

Glycine is an amino acid that helps build proteins needed for tissue and hormone maintenance. More glycine may help support heart and liver health, improve sleep, reduce diabetes risk, and reduce muscle loss. It is the main amino acid in collagen. Glycine is one of three amino acids that your body uses to make glutathione, a powerful antioxidant that helps protect your cells against oxidative damage caused by free radicals, which are thought to underlie many diseases.

The word “biotin” comes from the ancient Greek word “biotos,” which means “life” or “sustenance.” B vitamins, and specifically biotin, help keep your skin, hair, eyes, liver, and nervous system healthy. Biotin is also a crucial nutrient during pregnancy, as it’s important for embryonic growth.

Most people get the biotin they need from eating a healthy diet, but there have been many claims that getting more biotin can regulate your blood sugar, promote healthy hair, skin, and nails, and help pregnant moms have healthier babies. 

Note: Women who are pregnant or may become pregnant should not use finasteride or Dutasteride or handle the crushed or broken tablets. Finasteride can cause birth defects in male babies.

Mechanism of action. Through an unclear mechanism, minoxidil enhances hair growth by enlarging miniaturized hair follicles, extending anagen, and shortening telogen.

Finasteride, a competitive inhibitor of type II 5-alpha reductase, decreases production of dihydroxytestosterone. Oral finasteride reduces scalp dihydroxytestosterone levels by approximately 60% to 70%, depending on the dosage.

Finasteride has been shown to increase the hair count, physician-assessed hair coverage, and hair mass compared with placebo.

Finasteride can result in lower prostate-specific antigen levels in men, which should be taken into consideration when interpreting laboratory results. Due to hepatic metabolism of finasteride, precautions should be taken with patients with liver disease. With chronic treatment, finasteride may cause mild to moderate elevations in serum liver enzymes, but this is usually self-limiting and rarely requires dose modification or drug discontinuation.

Dutasteride, a potent type I and type II 5-alpha reductase inhibitor, is used to treat benign prostatic hyperplasia, but is also prescribed as an off-label treatment for pattern hair loss.

Latanoprost is a prostaglandin F2 analogue that extends the anagen phase. A double-blind, placebo-controlled, randomized trial in 16 men demonstrated an increase in hair density when latanoprost 0.1% was applied to a small area of the scalp.

Ketoconazole, an antifungal medication that has anti-inflammatory and antiandrogenetic properties was found to reduce inflammation in pattern hair loss and to benefit women with hyperandrogenemia and female pattern hair loss.

Spironolactone competitively inhibits the androgen receptor and also inhibits ovarian production of androgens. Of the antiandrogen drugs used off-label, it is the one most commonly used. It has been used to treat female pattern hair loss for over 2 decades and has a good safety profile.

Naltrexone/bupropion combines an opioid receptor antagonist (naltrexone) with a dopamine and norepinephrine reuptake inhibitor (bupropion). It is labeled as an adjunct to increased physical activity and a reduced-calorie diet for chronic weight management in adults.  These are also ingredients in the popular weight loss medication, Contrave.  They are associated with approximately 5-10%weight loss. However, it is not recommended for patients who have Hypertension, Seizures, Severe Depression, Suicidal ideations or history of suicidal behavior, or mania as Buproprion can make these symptoms worse, or patients taking chronic opioids (even Tramadol), as the naltrexone can cause withdrawal symptoms.  As a patient, you must have listed all of your current medications and medical history in your chart prior to approval of these medications as well as acknowledge that you have read these warnings. 

Metformin helps reduce your appetite. It does this in part by increasing levels of hormones that help you feel less hungry. It also:

• Limits how your liver produces glucose so that you may need less insulin
• Improves insulin resistance so the insulin your body produces works better 
• Changes your gut microbiome, the microorganisms in your digestive system that help break down food

Topiramate can speed up your metabolism, suppress your appetite, and reduces the excitation of the nerves in your brain's addiction centers (hence that sugar addiction!). It also reduces the amount of leptin your body produces. Leptin is known as the hunger hormone and tells you that you need to eat..

Bremelanotide injection is used to treat hypoactive sexual desire disorder (HSDD) in premenopausal women who previously had no problems with sexual desire and that occurs in any type of stimulation, situation, or partner. It should only be used in women who have low sexual desire that is troubling to them and is not caused by a medical or mental health problem, problems in the relationship, or medicine or other drug use. 

Women who have gone through menopause and men should not use bremelanotide. This would not be for any patient with uncontrolled hypertension.

Taldalfil works by increasing muscle relaxation, and targets muscle cells in blood vessels. When the blood vessel muscle cells relax, this expands the vessel and increases blood flow; this effect is referred to as vasodilation. This aids in the treatment of sexual dysfunction.  Traditionally it has been used to treat erectile dysfunction in men, but research shows that it may also benefit women. 

However, it has also been shown in studies to improve insulin sensitivity and lean muscle mass. Recent medical studies have suggested that using Tadalafil for weight loss might stimulate lipolysis, and potentially lead to increased fat utilization. So, the vasodilatory effects of Tadalafil improve flow of blood to muscles in the body and provide their capacity for energy expenditure during physical activity. In this way, Tadalafil for weight loss can impact indirectly.

L-arginine is an amino acid that helps the body build protein. Because L-arginine acts as a vasodilator, opening (dilating) blood vessels, many people use L-arginine to treat heart conditions and sexual dysfunction.

L-arginine can worsen allergies or asthma. Use the supplement with caution if you have these conditions.

Be careful about taking L-arginine if you've had cold sores or genital herpes (HSV). Too much L-arginine in your system can potentially trigger the virus that causes those conditions.

Retatrutide is revolutionary new drug currently in phase 3 of clinical trials on its way to FDA approval that takes action on three receptors in the body that leads to weight loss. These three receptors are GLP-1, GIP, and Glucagon. This can have additional benefits on appetite control and fat burning.
