With chronic treatment, finasteride may cause mild to moderate elevations in serum liver enzymes, but this is usually self-limiting and rarely requires dose modification or drug discontinuation.
Dutasteride, a potent type I and type II 5-alpha reductase inhibitor, is used to treat benign prostatic hyperplasia, but is also prescribed as an off-label treatment for pattern hair loss.
Latanoprost is a prostaglandin F2 analogue that extends the anagen phase. A double-blind, placebo-controlled, randomized trial in 16 men demonstrated an increase in hair density when latanoprost 0.1% was applied to a small area of the scalp.
Ketoconazole, an antifungal medication that has anti-inflammatory and antiandrogenetic properties was found to reduce inflammation in pattern hair loss and to benefit women with hyperandrogenemia and female pattern hair loss.
Spironolactone competitively inhibits the androgen receptor and also inhibits ovarian production of androgens. Of the antiandrogen drugs used off-label, it is the one most commonly used. It has been used to treat female pattern hair loss for over 2 decades and has a good safety profile.
Naltrexone/bupropion combines an opioid receptor antagonist (naltrexone) with a dopamine and norepinephrine reuptake inhibitor (bupropion). It is labeled as an adjunct to increased physical activity and a reduced-calorie diet for chronic weight management in adults. These are also ingredients in the popular weight loss medication, Contrave. They are associated with approximately 5-10%weight loss. However, it is not recommended for patients who have Hypertension, Seizures, Severe Depression, Suicidal ideations or history of suicidal behavior, or mania as Buproprion can make these symptoms worse, or patients taking chronic opioids (even Tramadol), as the naltrexone can cause withdrawal symptoms. As a patient, you must have listed all of your current medications and medical history in your chart prior to approval of these medications as well as acknowledge that you have read these warnings.

Women who have gone through menopause and men should not use bremelanotide. This would not be for any patient with uncontrolled hypertension.
Taldalfil works by increasing muscle relaxation, and targets muscle cells in blood vessels. When the blood vessel muscle cells relax, this expands the vessel and increases blood flow; this effect is referred to as vasodilation. This aids in the treatment of sexual dysfunction. Traditionally it has been used to treat erectile dysfunction in men, but research shows that it may also benefit women.
However, it has also been shown in studies to improve insulin sensitivity and lean muscle mass. Recent medical studies have suggested that using Tadalafil for weight loss might stimulate lipolysis, and potentially lead to increased fat utilization.
