How Does It Work?
Once you read all of the information on this page, and you decide you would like to trust me with your care, you will sign up as a new patient by clicking on the button below "Click to Join my Weight loss Program". You will be sent an intake form by email. Once you fill out this intake form and click submit, you will then be emailed a link to your patient portal. I will receive a notification of your intake form submission and subsequently review your intake form. If you are accepted as a new patient after review, you will receive further instructions via your patient portal. If for some rare reason you are not appropriate for my program, you will receive notification as well as a full refund.
Fees
There is an initial fee of $99.99 which includes my fee to become a new patient in my practice as well as to review your information and intake form.
After this inital fee, if you are desiring compounded medications, there are no additional monthly fees. All of the compounded medications/shipping directly to you/supplies/my fee are all inclusive and listed under the products site on this website (currently under re-construction and will be available again very soon).
If you are desirous of Brand name medications, there is a prescribing fee which includes the review of your chart, a virtual patient visit with you, and prescribing the medication. This fee is 49.99 for a 4-week prescription sent to your local pharmacy. You will then pay for your prescribed medication at your local pharmacy. Once you are on maintenance, a 12-week prescription can be sent for $99.99 if desired. These fees are for all Brand name medications ONLY.
Your BMI must be at least 30 to qualify for Mounjaro, Zepbound, Wegovy, Ozempic, tirzepatide, and semaglutide. However, you may be considered for these medications if your BMI is 27-29.9 AND you have been diagnosed with high blood pressure, high cholesterol, sleep apnea, severe asthma/COPD, heart disease, or diabetes. Please upload any supporting documentation to show that you have been diagnosed with any of these conditions. If you have already been on an injectable weight loss medication prescribed by another provider, your current BMI may be lower than 30, and that's acceptable. However, your starting BMI must have met the same criteria above to qualify for the program and continue your weight loss journey with my practice.

Where are you located? My practice is by Telemedicine only. Your patient visits will be by virtual visit. I am licensed to practice medicine in Alabama and Tennessee.
