Affordable Weight Loss Fees
I try to keep your fees as low as possible.
For BRAND NAME Medications (Mounjaro/Ozempic/Wegovy):
Initial visits currently start at $79.99, and monthly fees are $49.99. Once you reach the maximum dose, you will be eligible to pay discounted quarterly dues of $99.99. Your medication is paid for separately at the pharmacy. This fee is for brand-named medications you pick up from your local pharmacy.
For Zepbound:
Initial visits currently start at $79.99, and monthly fees are $79.99. Once you reach the maximum dose, you will be eligible to pay discounted quarterly dues of $149.99. Your medication is paid for separately at the pharmacy. This fee is for brand-named medications you pick up from your local pharmacy.
*OR*
For COMPOUNDED Medications:
Fees for semaglutide start at $200/month and tirzepatide starts at $300/month, all fees included (even the medication) with free shipping to your home!* These medications are from my preferred compounding pharmacies.
*NEW PATIENTS will pay an initial $79.99 set-up fee, then all fees are included in the monthly price.
The medications I prescribe are available in different doses. While everyone begins on the same starting dose, not everyone advances doses at the same rate. Some even stay on the starting dose. With our easy monthly refill program, you are able to customize your journey month by month.
I am currently available for consults in
ALABAMA and TENNESSEE
Your prescription must be sent to a pharmacy in this state or medication shipped to an address in this state. You must also physically be in this state when you request your medication, NO EXCEPTIONS! Those are the rules of telemedicine.
NEW PATIENT STEPS:
1) If you have not already done so, use the link on the first page to link to my Weight Loss practice and pay the new patient fee. If for some reason you are not accepted as a patient after reviewing your intake forms, you will be REFUNDED this fee.
2) Please ensure to input a LOCAL pharmacy when prompted on your intake form. This will be the pharmacy that will be used for any medications NOT shipped to you from the compounding pharmacy. This will also be the pharmacy I send in any nausea medications if needed. Please fill out this part of your intake form.
3) Please completely fill out your intake form including medical history and any current medications and medication allergies. Take a picture of your ID and upload it. Please include important details such as your current height and weight (or BMI), any weight loss medications you are currently on or have tried in the past, etc.
I will send you the New Patient Information Packet via your PATIENT PORTAL. You MUST activate your patient portal after filling out your intake form to ensure you receive your approval and instructions. Be sure and read your NEW PATIENT packet, as it contains a lot of valuable info and answers a lot of the commonly-asked questions, such as how to order, how we communicate, and a facebook group to join alongside other clients.
7) Please review ALL medication details to help me decide which medication may be right for you. Your handout I send will explain in detail the different options. Please pay close attention to BMI requirements. I will discuss your choice and make recommendations for you.
8) You may request a prescription in your patient portal if you are wanting brand name medication (please make sure your insurance covers the medication you are requesting!). However, if you are interested in a medication on the PRODUCTS page, your order will be placed separately, depending on which medication you choose. My preferred pharmacy has their own system that you must be added to, and the orders are placed directly into their system.
9) I will review your chart and make sure all of your information and your ID is loaded. I will discuss any questions you may have about the medications. If approved (very few people are not approved), I will send in nausea medication to your local pharmacy that you selected (nausea is the most common side effect).
More details can be found in your New Patient Info Pack.
10) Please review each medication on this page to learn more. Please read all info regarding turn-around times and medication information. You may request up to a 12-week supply at a time of each medication within a certain timeframe.
As I continue to add more medications to the practice, the PRODUCTS page will have the most up-to-date information as far as products I offer and information about them.
If you do not want the nausea medication, simply let me know. However, I STRONGLY suggest that you do so you will have it on hand.
**In the RARE case that you are NOT approved, the order will be cancelled and your account will NOT be charged. The temporary $79.99 charge will disappear off your credit/debit card in a few days.**
ESTABLISHED PATIENTS:
You may simply send me a message in your patient portal if you would like a medication from the PRODUCTS page if you have already ordered a medication previously. If you are switching from brand-named Mounjaro or Wegovy, you are considered an "established patient" and DO NOT have to pay another onboarding fee.
ALL PATIENTS:
To ensure I get your messages and questions answered in a timely manner, PLEASE message me through your patient portal. If you have not yet become a patient, you may email me at yourprovider@shanamotonpa.com .
Please Allow 24 hours for an email response.
Becoming a weight loss patient does not negate the fees of Urgent Care or NFP if those services are needed. This is a completely separate practice from those services.
TIRZEPATIDE/L-Carnitine; TIRZEPATIDE/METHYLCOBALAMIN; TIRZEPATIDE/Niacinamide; Tirzepatide/B6; Tirzepatide/B12
My preferred pharmacies have mixed tirzepatide with L-Carnitine; Tirzepatide with Methylcobalamin (Methylated B12); Tirzepatide with Niacinamide (B3); Tirzepatide with B6; and Tirzepatide with B12.
For patients who have NOT previously been on any weight-loss medications, you will likely qualify for one of the medications in my program if you are a generally healthy person between the ages of 18-64 with a BMI of 30+. Patients with risk factors such as high blood pressure, high cholesterol, Type 2 Diabetes, or sleep apnea may qualify with a BMI of 27+. Patients aged 65-69, please ask for an exception. If you are 70+, you are best served by your primary care physician for weigh loss options.
Once you are approaching your goal weight and have a BMI under 25, please contact me for information on maintenance dosing.
*DO NOT use any of the injectable medications if you or any of your family members have ever had a type of thyroid cancer called MTC or if you have an endocrine system condition called MEN 2, or if you have had a serious allergic reaction to tirzepatide or semaglutide.
*For ladies on oral birth control pills, these meds can change the way your body absorbs certain medications, and the pills may not work as well. It is recommended that you use a back-up birth control method to prevent pregnancy when first starting a new dose. See the website for more info.
These injections can cause serious side effects in certain people including nausea, vomiting, diarrhea, and constipation..
DOSING: Please see the PRODUCTS page for dosing. Be sure to follow the instructions on your vial or the order site. All patients new to TZ MUST start at the 2.5mg dose then you may increase your dose each 4-weeks if desired. If you have previously been on TZ with another prescriber, we can start at a higher dose with proof that you have completed the lower dose.
SEMAGLUTIDE/L-Carnitine and SEMAGLUTIDE/B12 and Pure Semaglutide
In most studies, tirzepatide was superior to semaglutide for weight loss. However, semaglutide is an excellent alternative for patients who need a medication with a lower cost, or who cannot tolerate tirzepatide. Please review the information found at www.drugs.com/semaglutide.html.

PRICING AND DOSING FOR SEMAGLUTIDE/Combos and Tirzepatide/Combos
Prices include my fees, the medication, supplies, and shipping to your home. Each dose comes in a multi-use vial. Each vial from my national pharmacy has 4 doses to last 4 weeks . The starting dose is 0.25 mg for patients that have not been on one of the injectable medications. You may increase the dose every 4 weeks if desired. If you are doing well on a dose, you may remain on that dose and not move up. The doses are 0.25mg, 0.5mg, 1mg, 1.7, and 2.5mg. The dose in units a week will be on your vial. The medication is more concentrated as you move up in dosage.
If you are currently switching from another injectable medication, we can start you on a higher dose:
1) Patients currently on 2.5 or 5mg tirzepatide may start at the 1mg dose. Patients on 7.5mg may start at the 1.7mg dose, and patients on the 10mg dose or higher of tirzepatide may start at the max dose of 2.5mg semaglutide.
2) Patients currently on semaglutide with another provider may start at the same dose of semaglutide or one dose higher. You may also, of course, go down a dose.
3) It is VERY important that you follow these guidelines to avoid side effects. Your body needs to adjust to a smaller dose before you are ready for the higher doses.
4) Tirzepatide dosing starts at 2.5mg, then 5mg, 7.5mg, 10mg, 12.5mg, 15mg
